By checking this box and submitting this reservation, I understand that I am responsible for payment in the amount shown on the "lesson length" line above. I understand that any cancellation requires 48 hour notice, and that cancellations within this time may be charged at the full rate. Any late show 15 minutes past the noted registration time may force the lesson to be cancelled. To assure the best possible outcome for all parties, I will inform the instructor as soon as possible concerning any changes or cancellations.
